About the Event:
TOPIC: Health Care Transition for Youth and Young Adults with Special Health Care Needs
Our speaker will be Antoinette W. Coward, Health Care Transition Coordinator for the Maryland DHMH Office for Genetics and People with Special Health Care Needs.
In her capacity at the Office for Genetics and People with Special Health Care Needs, Ms. Coward coordinates the part of its mission to ensure that all Maryland youth with special health care needs receive the services necessary to make successful transitions to all aspects of adult life, including adult health care, employment and independence.
During this lecture, Ms. Coward will share her in-depth experience and knowledge of transition-related educational, service and outreach activities and resources.

The Adolescent Transition Lectures are a series of presentations and web conferences designed to educate individuals with disabilities, parents, clinicians and community providers about issues important to adolescents and young adults with disabilities approaching adulthood. The lectures are intended to provoke and facilitate discussion.
