Referral specialists are available from 8:30 a.m. to 5:00 p.m., Eastern Standard Time (EST), Monday through Friday, and will gladly answer your questions. If you reach voice mail, please leave a message and a referral specialist will promptly return your call.

If you would like to request an appointment online, all information requested in this process will be kept strictly confidential. 

You must complete an Intake in order to have your child seen within the CARD outpatient clinic.

Once the Intake process is completed, your child’s information will be sent to the CARD outpatient clinic. Additional information will be requested from you in order to complete the intake process. After your child’s information is reviewed, you will receive a call, as well as an email or letter detailing the services for which your child has been recommended.

Your child will be scheduled for the most appropriate services in the order in which their referral was received. One of our Care Center Specialists will contact you to schedule a future appointment.

In an effort to provide the most comprehensive service possible, you will be asked to fill out the “CARD Background and History Form” prior to your appointment. The form will be sent to you after your child’s appointment is scheduled. This information will help us to better serve your child at our center.
