You must call the number above and complete an Intake in order to have your child seen within the CARD outpatient clinic.
Once the Intake process is completed, your child’s information will be sent to the CARD outpatient clinic. Additional information may be needed in order to complete the intake process. After your intake is reviewed, you will receive a letter detailing the services for which your child has been recommended.
Please be advised that most of our services do involve a wait time, and that wait time will vary based on the services recommended for each child. One of our Care Center Specialists will contact you to schedule a future appointment when your child’s name is next on our waitlist.
We are experiencing a significant increase in volume of new patient referrals and are working diligently to schedule every child as soon as possible. However, due to the increase in referrals, our timeline for scheduling your appointment may be subject to change.

Please complete and return this form to us at soon as possible so that your child’s service provider has adequate time to review the information prior to your child’s appointment. If you have any technical difficulties with this electronic form, please print this PDF version of the form.
